Lost a CE certificate? Recover the proof before you count the hours.
A course log can tell you what is missing, but it cannot replace completion proof. Start with the provider that issued the course, request a completed duplicate, and check the recovered document against the live BRN rules and your own notice.

Include the likely course title and date, the name used at enrollment, and any receipt or completion email that helps locate the record.
Do not substitute
A row in a tracker is not the certificate.
Keep the row as an index, then link it to the actual completion document after the provider supplies the replacement.

Identify the exact course
Find the course title, completion date, provider name, payment receipt, LMS record, or confirmation email. These clues help the provider locate the completion record.
Ask the provider for a duplicate
Request a completed replacement certificate or accepted proof of attendance. Do not ask for a blank certificate and do not invent missing provider or hour information.
Check the replacement fields
Confirm the learner identifier, course title, provider details, completion date, contact hours, signature, and retention statement before relying on the replacement.
Reconcile the renewal window
Match the recovered proof to the period in your BreEZe record or audit notice. A recovered certificate does not make an out-of-window or otherwise ineligible course count.
Duplicate-certificate request
Give the provider enough detail to find the record.
Send this only to the actual course provider. Do not include a license number, birth date, or other sensitive identifier unless the provider securely requests information needed to match its own record.
After the proof is recovered
Check it, then place it in the renewal record.

Can a spreadsheet replace a lost California RN CE certificate?

What if an audit deadline is already running?
Read the individual notice and contact the California BRN Continuing Education Program when the requested proof cannot be obtained in time. Do not assume a generic deadline or substitute an organizer for the missing proof.
