What each column is for
Course
Use the course title printed on the completion record.
Provider
Record the continuing-education provider name.
CEP number
Record the number, then check its current status through BreEZe.
Completion date
Use the date on the completion record so you can reconcile the applicable renewal period.
Contact hours
Enter the awarded hours exactly; the log does not decide whether a course is eligible.
The log is an index, not proof
Keep the original certificate or grade slip for four years.
California BRN guidance says 30 contact hours are generally required every two years for an active RN license and the supporting certificates or grade slips must be retained for four years. Confirm the live rule, provider status, and any first-renewal or advanced-practice exception before relying on your total.

Does the log replace my completion certificates?
No. The California BRN says RNs must retain certificates or grade slips for four years as proof in the event of an audit.
