What the official rule asks for
CCR §1458 describes the completion document—not a field you should repair yourself.
For a certificate or diploma documenting successful completion, CCR §1458 lists the learner name and RN license or other identification number, course title, provider name, address and provider number, course date, contact hours, and an instructor/provider/designee signature. It also requires the licensee to retain the document for four years.
Missing contact hours
Ask the issuing provider to reissue or correct the completion document so it states the number of CE contact hours awarded. Do not calculate a number and add it to the certificate yourself.
Missing provider number or accreditor identifier
Ask which approval or accreditation applies to this course and how it should appear on the completion proof. Do not copy a number from an unrelated course or search result.
Missing signature
CCR §1458 calls for the signature of the instructor, provider, or provider designee. Ask the issuer for a signed or properly authenticated replacement rather than signing it yourself.
A missing California CEP number is not, by itself, enough for this page to decide eligibility. BRN guidance describes other approval and academic-course situations. Confirm the applicable provider or accreditor basis through the official source and the issuer; never guess a number.

Can I type missing contact hours onto my California RN CE certificate?
No. Ask the issuing provider for a corrected or reissued completion document. ShiftProof does not create, alter, or validate provider certificates.
Does a missing California CEP number always mean the course cannot count?
Not necessarily. California BRN guidance describes circumstances involving other health-professional approvals, academic courses, and certain out-of-state courses. Confirm which provider or accreditor basis applies instead of inventing a CEP number.
Can an invoice or LMS screenshot replace the incomplete certificate?
A receipt or dashboard can help the provider locate a record, but it does not automatically supply the completion information required by CCR §1458. Follow any BRN notice and request proper completion proof from the issuer.
What if the provider cannot correct the certificate before an audit deadline?
Follow the exact deadline and instructions in the individual notice, keep a record of the replacement request, and contact the California BRN about the unresolved proof. Do not assume a generic extension or substitute fabricated information.
